xác suất bị "long covid"
#1

Predicting Long COVID at initial point of COVID-19 diagnosis: Study finds several warning factors

Date: January 25, 2022

Source: Institute for Systems Biology

Summary: A study has identified four predictive factors of Post Acute Sequelae of COVID-19 (PASC), often called long COVID. These 'PASC factors' can be identified at the initial point of COVID-19 diagnosis and can anticipate if a patient is likely to develop long COVID. Additionally, researchers found that mild cases of COVID-19, not just severe cases, are associated with long COVID, and that administering antivirals very early in the disease course may potentially prevent some PASC.

A significant portion of people who contract the SARS-CoV-2 virus -- some estimates suggest more than 40 percent -- suffer chronic effects known as Post Acute Sequelae of COVID-19 (PASC), commonly referred to as long COVID. PASC symptoms include fatigue, brain fog, the loss of taste and smell, shortness of breath, and more.

Now, researchers have identified several factors that can be measured at the initial point of COVID-19 diagnosis that anticipate if a patient is likely to develop long COVID. These "PASC factors" are the presence of certain autoantibodies, pre-existing Type 2 diabetes, SARS-CoV-2 RNA levels in the blood, and Epstein-Barr virus DNA levels in blood.

"Identifying these PASC factors is a major step forward for not only understanding long COVID and potentially treating it, but also which patients are at highest risk for the development of chronic conditions," said ISB President, Dr. Jim Heath, co-corresponding author of a research paper that will be published by the journal Cell. "These findings are also helping us frame our thinking around other chronic conditions, such as post-acute Lyme syndrome, for example."
Additionally, researchers found that mild cases of COVID-19, not just severe cases, are associated with long COVID. They also suggest that administering antivirals very early in the disease course may potentially prevent some PASC.

"Long COVID is causing significant morbidity in survivors of COVID-19, yet the pathobiology is poorly understood," said Dr. Jason Goldman, co-corresponding author of the paper and an infectious disease expert at Swedish. "Our study pairs clinical data and patient-reported outcomes with deep multi-omic analyses to unravel important biological associations that occur in patients with PASC. Certain findings such as the low cortisol state in patients with long COVID have potential to translate rapidly to the clinic. Our results form an important foundation for the development of therapeutics to treat long COVID."

Researchers collected blood and swab samples from 309 COVID-19 patients at different time points to perform comprehensive phenotyping which was integrated with clinical data and patient-reported symptoms to carry out a deep multi-omic, longitudinal investigation.

A key finding from the study deals with viral load, which can be measured near diagnosis to predict long COVID symptoms. "We found that early blood viral measurements are strongly associated with certain long COVID symptoms that patients will develop months later," said Dr. Yapeng Su, a co-first and co-corresponding author of the paper.

In addition, researchers found the Epstein-Barr virus (EBV) -- a virus that infects 90 percent of the human population and is normally inactive in the body after infection -- is reactivated early on after SARS-CoV-2 infection, which is significantly associated with future long COVID symptoms. "This may be related to immune dysregulation during COVID-19 infection," Su added.

The team also found that PASC is anticipated by autoantibodies (which associate with autoimmune diseases like lupus) at diagnosis, and that as autoantibodies increase, protective SARS-CoV-2 antibodies decrease. This suggests a relationship between long COVID, autoantibodies and patients at elevated risk of re-infections.

"Many patients with high autoantibodies simultaneously have low (protective) antibodies that neutralize SARS-CoV-2, and that's going to make them more susceptible to breakthrough infections," said Daniel Chen, a co-first author of the paper.

The research project was a collaboration between ISB, Providence, Swedish the University of Washington, Fred Hutchinson Cancer Research Center, Stanford, UCLA, UCSF, and others.

Long Covid: What is it and what are the symptoms?

The Office for National Statistics (ONS) estimates about 1.3 million people in the UK have "long Covid" - symptoms lasting more than four weeks.

Most people who catch Covid don't become severely ill and get better relatively quickly.
But some have long-term problems after recovering from the original infection - even if they weren't very ill in the first place.

What are long Covid symptoms?

Long Covid is not fully understood, and there is no internationally-agreed definition - so measures of how common it is or what symptoms are involved vary.

Guidance for UK health workers describes long Covid as symptoms that continue for more than 12 weeks after an infection which cannot be explained by another cause.

According to the NHS website, these can include:
  • extreme tiredness
  • shortness of breath, heart palpitations, chest pain or tightness
  • problems with memory and concentration ("brain fog")
  • changes to taste and smell
  • joint pain
But patient surveys have identified tens and even hundreds of other symptoms. A large study by University College London (UCL) identified 200 symptoms affecting 10 organ systems.

These include hallucinations, insomnia, hearing and vision changes, short-term memory loss and speech and language issues. Others have reported gastro-intestinal and bladder problems, changes to periods and skin conditions.

The severity of symptoms varies, but many have been left unable to perform tasks like showering or remembering words. However these symptoms can have other causes too.

ONS research published in September 2021 found 0.5% of people who tested negative for coronavirus had at least one symptom that lasted for three months, compared to 3% who tested positive.

Until then .... Good Luck....

German vaccine body backs fourth COVID-19 shot for most vulnerable

Germany's vaccination commission has recommended second booster shots for people in coronavirus at-risk groups, including those aged over 70 or with immunodeficiencies.

Germany's expert vaccine commission, the Standing Committee on Vaccination (STIKO), came out on Thursday in favor of fourth COVID-19 vaccine doses — or second booster shots — for people belonging to particularly at-risk groups.

The commission recommended that people aged 70 and over, people with immunodeficiency, and people who live in in care facilities, as well as employees of care and medical facilities, should all be given a fourth dose.

STIKO reasoned that the waning protection offered by existing vaccines against the virus means that a second booster after six months — with vaccines that are already on hand in Germany — would help protect the most vulnerable.

German Health Minister Karl Lauterbach welcomed STIKO's decision, calling it "correct" and saying it "gives added protection to [older people and at-risk patients]."
"Nevertheless, first vaccines and boosters should remain our priority," the minister added.

Following in Israel's footsteps

"The latest data from Israel shows that a fourth dose can provide a slight improvement in protection against infection and notable protection from a serious case of illness," STIKO head Thomas Mertens told German newspapers under the umbrella of the Funke Media Group publisher ahead of the announcement on Thursday.

Several pharmaceutical companies, such as Moderna and Germany's BioNTech, have said they will make a new shot that is more effective against the highly infectious omicron variant of the virus. Mertens said the STIKO was awaiting clinical data from these companies on the new omicron-adapted vaccines.

In January, Israeli Prime Minister Naftali Bennett said a fourth COVID-19 shot would be made available to medical workers, along with people aged 60 and older. Hundreds of thousands of people in the Mediterranean country have already received a fourth jab so far, although one Israeli study suggested another shot might be less effective against omicron.

The STIKO also noted in its announcement that "the data on the effectiveness and safety of a second booster vaccination is still limited."

Is a fourth shot necessary yet in Germany?

It's unclear, however, whether such an approach will work in Germany, where vaccine hesitancy remains strong. So far, around 74% of Germans are vaccinated at least twice, a figure lower than other European countries such as France and Spain. Various politicians, including Health Minister Lauterbach, have repeatedly said the main priority is to improve that quota.

STIKO also recommended the use of the Novavax vaccine on Thursday. This one is based on more conventional vaccine technology and it is hoped that some people who have been hesitant to receive already approved vaccines may accept the Novavax one.

German lawmakers are discussing making COVID-19 vaccination obligatory to fight the pandemic, but it's unclear whether the idea will become public policy. Cases have surged in recent weeks, with Germany reporting a record 236,120 new infections on Thursday and a seven-day incidence of more than 1,220 per 100,000.

Beyond that, Germany's vaccination cycle began considerably later than Israel's, meaning most people who have so far received three shots would have likely had their initial "booster" later than a typical Israeli.

Differing opinions over the benefits of boosters

The Greens' health policy expert, Janosch Dahmen, told Funke newspapers that a fourth dose would probably be advisable for people with preexisting immune system concerns or who rely on medication that can compromise their immune system. But he said for many Germans it could also prove unnecessary, at least at present.

"The vast majority of people in Germany who have received a booster vaccination did so in December or January. Their immune protection against serious bouts of COVID is currently very good," Dahmen said.

Global health authorities, such as the World Health Organization (WHO), have condemned the use of booster shots by wealthy countries. The WHO has said the booster approach is contributing to global vaccine inequity, particularly in Africa.

California will lift mask mandate for vaccinated residents in indoor public places next week

SAN FRANCISCO — With the Omicron coronavirus surge rapidly receding, California will lift its universal mask mandate for indoor public places next week, state officials announced Monday.
The lifting of the mandate will apply to counties without local mask orders of their own, such as San Diego, Orange, Riverside and San Bernardino counties, as well as swaths of the San Joaquin Valley. Counties can still opt to retain local mask orders, as Los Angeles County will do.

While this move doesn’t mean face coverings will be a thing of the past — they still will be required for unvaccinated residents indoors and for everyone in select settings, such as nursing homes or while aboard public transit — relaxing the roughly two-month-old order reflects the progress California has made in its battle against Omicron, even as officials say continued vigilance will be vital in keeping the state on the right track.
